Hamad Medical Corporation (HMC) Exemption

Procedure for obtaining HMC Exemption:

1. Obtain a medical certificate from an HMC physician addressed to the ‘HMC Handicap Exemption Committee’.
2. Complete the Health Card Form. Parents must fill out the health card form with the assistance of a social worker at HMC who will provide support throughout documentation process.
3. Submission of documents to the HMC Handicap Exemption Committee.
4. Exemption approval: If the application is approved by the committee, the family will be exempted from paying for appointments at HMC or PHCC (Primar Health Corporation )

N.B: This exemption does not apply to care at Sidra Medicine.

Documents required:

1. Medical certificate signed by consultant (Medical certificate must be from HMC).
2.  Copy of mother passport & QID.
3. Copy of father passport & QID.
4. Copy of patient passport & QID.
5. Copy of patient health card.
