Referral Guidelines

A brief overview of the steps healthcare professionals can take in order to refer a patient to the Sidra Outpatient Clinic

For assistance with Referrals or queries from physician offices regarding referrals, please call 4003-3333 and ask to be connected to the Referral Center.

Follow up appointments can be made prior to a patient leaving clinic or the patient may call Sidra at 4003-3333 to schedule a follow up appointment.

PHCC to Sidra

  • Step 1

    Complete HMC Referral Request Form and write “SIDRA” at top of the form.

    Ensure Referring Physician’s name and location are identified.

  • Step 2

    Submit Referral Request Form per established process for your facility.

    Sidra Referral Request Forms will be sent to Sidra Referral Center.

  • Step 3

    PHCC Referrals are reviewed and selected referrals sent to Sidra Referral Center Staff.

    PHCC referred patients who meet Sidra criteria are identified.

  • Step 4

    Sidra staff call identified patients to schedule an appointment. 

    Patient receives immediate confirmation of appointment by SMS and a follow up reminder sent 24 – 48 hrs. prior to the appointment.

  • Step 5

    Patient Visit Occurs.

    A copy of the Sidra Clinical Summary is scanned into the   patient’s HMC EMR.

HMC to Sidra (paper form)

  • Step 1

    Complete HMC Referral Request Form and write “SIDRA” at top of the form.

    Ensure Referring Physician’s name and location are identified.

  • Step 2

    Submit Referral Request Form per established process for your facility.

    Sidra Referral Request Forms will be couriered to the RBMS at Barwa Tower.

  • Step 3

    HMC Referrals are reviewed and selected referrals sent to Sidra Referral Center Staff.

    HMC referred patients who meet Sidra criteria are identified.  Paper Referral Forms are couriered to the Sidra Referral Center.

  • Step 4

    Sidra staff call identified patients to schedule an appointment. 

    Patient receives immediate confirmation of appointment by SMS and a follow up reminder sent 24 – 48 hrs. prior to the appointment.

  • Step 5

    Patient Visit Occurs.

    A copy of the Sidra Clinical Summary is scanned into the   patient’s HMC EMR.

HMC to Sidra (e-Referrals)

  • Step 1

    From “Orders” select New

    Enter Referral to Sidra Pediatric Services

    From drop down select the Desired Service

    Complete requested fields in the order

  • Step 2

    Sign Order

  • Step 3

    Sidra Staff will obtain electronic referral list and determine acceptance based on Sidra criteria

  • Step 4

    Sidra staff call identified patients to schedule an appointment. 

    Patient receives immediate confirmation of appointment by SMS and a follow up reminder sent 24 – 48 hrs. prior to the appointment.

  • Step 5

    Patient Visit Occurs.

    A copy of the Sidra Clinical Summary is scanned into the  patient’s HMC EMR.

PEC to Sidra

  • Step 1

    Complete HMC Referral Request Form and write “SIDRA” at top of the form.

    Ensure Referring Physician’s name and location are identified.

  • Step 2

    Submit Referral Request Form per established process for your facility.

    Sidra Referral Request Forms will be couriered to the RBMS at Barwa Tower.

  • Step 3

    HMC Referrals are reviewed and selected referrals sent to Sidra Referral Center Staff.

    PEC referred patients who meet Sidra criteria are identified.  Paper Referral Forms are couriered to the Sidra Referral Center.

  • Step 4

    Sidra staff call identified patients to schedule an appointment. 

    Patient receives immediate confirmation of appointment by SMS and a follow up reminder sent 24 – 48 hrs. prior to the appointment.

  • Step 5

    Patient Visit Occurs.

    A copy of the Sidra Clinical Summary is scanned into the   patient’s HMC EMR.

QF – PHCC (clinic) to Sidra

  • Step 1

    Complete Sidra Referral Request Form

    Ensure Referring Physician’s name and location are identified.

  • Step 2

    Submit Referral Request Form per established process for your facility.

    Sidra Referral Request forms will be sent via Lab courier to Sidra Referral Center.

     

  • Step 3

    QF-PHCC Referrals for Sidra are vetted by Sidra Physician(s).

    QF-PHCC referred patients who meet Sidra criteria are identified.

     

  • Step 4

    Sidra staff call identified patients to schedule an appointment. 

    Patient receives immediate confirmation of appointment by SMS and a follow up reminder sent 24 – 48 hrs. prior to the appointment.

  • Step 5

    Patient Visit Occurs.

    A copy of the Sidra Clinical Summary is scanned into the   patient’s HMC EMR.
